Coverage in Alberta

Alberta residents aged 18 and over with diabetes and are enrolled in a government-sponsored health benefit plan are eligible for provincial reimbursement for the FreeStyle Libre 2 system if they require ongoing use of an insulin pump, or use both a basal and bolus insulin, or a premixed insulin. Alberta residents under the age of 18 years with diabetes that require ongoing use of any insulin or insulin pump therapy are also eligible for provincial reimbursement of the FreeStyle Libre 2 system.

Coverage in Ontario

The FreeStyle Libre 2 system is reimbursed under Ontario Drug Benefit (ODB) program for people managing diabetes with insulin. Patients managing their diabetes with insulin are eligible for a maximum quantity of 33 sensors over the course of a 365-day period.

Coverage in Quebec

The FreeStyle Libre 2 system is reimbursed under Régie de l'assurance maladie du Québec (RAMQ) for individuals under the age of 18 years living with Type 1 diabetes or people 18 years and over who are treated with intensive insulin therapy (treatment by insulin pump or ≥ 3 insulin injections per day) and meet specific criteria*.

Coverage in Saskatchewan

The FreeStyle Libre 2 system is reimbursed under the Saskatchewan Extended Benefits and Drug Plan for pediatric patients between the ages of 4 and 17 years living with diabetes that meet special criteria*.

Coverage in Manitoba

The FreeStyle Libre 2 system is reimbursed under the Manitoba Pharmacare Program for people living with Type 1 or Type 2 diabetes currently on both basal and bolus insulin or using an insulin pump. Patients managing their diabetes with insulin are eligible for a maximum quantity of 33 sensors per benefit year.

The FreeStyle Libre 2 system is reimbursed under the Prince Edward Island Glucose Sensor Program for people diagnosed by a medical practitioner or nurse practitioner as having diabetes and are reliant on an insulin pump or multiple daily injections of insulin (i.e., 3 or more) to manage their diabetes.

The Yukon Chronic Disease and Disability Benefits program covers the FreeStyle Libre 2 system for individuals living with type 1 diabetes.

Coverage in the Northwest Territories

The FreeStyle Libre 2 system is reimbursed under the Government of Northwest Territories Seniors (Group 60), Specified Disease Conditions (Group 21992) and Métis Health Benefit (Group 19866) Programs for people who are 19 years of age and younger on intensive insulin therapy (administration of short-acting insulin three or more times per day), or clients with type 1 diabetes, as verified by short-acting insulin claims history*.

Non-Insured Health Benefits (NIHB) coverage

The FreeStyle Libre 2 system is reimbursed under the Non-Insured Health Benefits program as a limited use benefit for individuals managing diabetes with insulin. Coverage is limited to 1 reader every 3 years and 14 sensors every six months.

Coverage in British Columbia

The FreeStyle Libre 2 system is reimbursed under the BC Pharmacare program for people with diabetes mellitus who are: 4 years of age or older, require multiple daily injections of insulin or insulin pump therapy, and have one of the following symptoms: hypoglycemia unawareness, frequent and unpredictable hypoglycemic episodes, unpredictable swings in blood glucose, at least one functional restriction that inhibits the use of blood glucose test strips, an occupation where hypoglycemia presents a significant safety risk*.

Coverage in Nova Scotia

The FreeStyle Libre 2 system is reimbursed under the Sensor-based Glucose Monitoring Program and the Nova Scotia Pharmacare Program for individuals living with Type 1 or Type 2 diabetes who meet specific criteria*, including the need for multiple daily injections of insulin. “Multiple daily injections of insulin” is defined as 1 (or more) injection(s) of basal insulin and 3 (or more) injections of bolus insulin, with a minimum of at least 4 total insulin injections per day.

Coverage in New Brunswick

The FreeStyle Libre 2 system is covered under the New Brunswick Insulin Pump Program (NBIPP) for medically eligible* New Brunswickers living with Type 1 diabetes and with Type 2 diabetes on intensive insulin therapy, requiring 3 or more insulin injections per day.
